Just design one? I did, it's the best way to learn.

Just put in input fields for name, company, phone, date (make it read-only
and set to the value of "Now()").

And then I put in checkboxes such as:

- Telephoned
- Please call back
- Called in to see you
- Will call back
- Wants to arrange meeting
- Returned your call
- Personal call
- Wants info (cold call)

as some can be used in conjunction with others. Just customise to your
needs. Quite an easy form.
