Date and Time Picker 6.0

I have attempted to used the Date and Time Picker 6.0 (Excel 2013), but almost every time I close the document something apparently goes wrong. When I reopen the document, the Date selector has failed and shows up as a glitchy looking thing that is no longer functional.
