E-Mail shows attachment but has none

Hi,
why does Outlook show the attachment icon when there is no attachment with
that mail?
I could not make it up to an special setting or situation.

if the message is in rich text format and there's an image in it, it may show
the paper clip

sorry, but the e-mails are in html format. it is very confusing when it
shows the paper clip but there is no document attached.

any other idea?
