I need to design a form, which I can distribute to others (who will ultimately use the form as the basis for an email to me), that has the following characteristics: 1) a drop-down list of x items (from which to select), and 2) a bullet list for multiple item #s to be entered (to simplify cut-n-paste for me). I started designing the form, but ran into difficulty when attempting to create a bullet point list. Also, the process currently works as follows: coworkers will forward an email to me with the information I need in it. Is it possible for this form to be incorporated into the forwarded email, or can the form only be the content of a new message by itself?

Are there other existing threads in this forum that would help me in the design phase, or can you suggest another resource?
