How can create a report that looks like a flyer?

I have a group (ungrouped) of check boxes and a group radio buttons in a form
.. I need the report to create a flyer that contains each checked item (the
noun) and one of the radio buttons (the verb). Also include the comments,
date and time and the emp name who reated the record. I will need a report
for each record. The report needs to automatically be faxed out. AND if
possible, the form needs to pop up every hour to be filled out and faxed
out.I haven't used access since 1995 (window 3.1) so I am a bit rusty. I
have created the table and the form. I've been doing webdesign for the past
seven years, so I am used to creating my form (MS Frontpage) and
automatically generating the table. I finally realized in Access you have to
create the table first. I see mention of creating invoices which I figure I
could use as an example of creating a report for every record, but I do see
more than a general mention. Please help. thanx.
