How do you create a NEW menu Heading on a Custom Toolbar

I can see how to add commands by dragging and dropping them to the
custom toolbar from the commands tab - but can't find out how to create
a new menu heading, to allow me to drop commands into this menu
heading.

The very last item in the list of commands on the left is "New Menu".
