Messages appear in sent items but are not sent

Problem Outline;

Environment; Exch 5.5 sp4, Win NT 4.0 sp6 server, Win XP sp2 clients
running Office XP sp3.

1. User sends email to client and cc's others both inside and outside the
office.
2. Email appears in sent items folder but is not delivered to the "To"
addressee outside the office or any of the CC's if the CC's contain both
internal and external addresses.

This has happened 3 times to the same user (sending to the same group of
clients/associates). I've read about the issues with contact records and
autocomplete, but I don't think any of those apply here.

Anyone got any ideas?
