Operation Failed message when attempting to create new msg or repl

I have a user that cannot send or reply to messages in Outlook 2003. She
initially had an error message referencing the outlvba.dll file (invalid
addin) whenever she launched Outlook. After closing the error message,
Outlook loaded ok, but whenever she attempts to create a new message, or
reply, she gets, "Operation Failed".

I have deselected LDVP from the Add-Ins and the (ooutlvba.dll) error went
away, but the "Operation Failed" error will not. I have had her run "Detect
and Repair" to no resolve.
