How can I create a file with birthdays, lets say, to be imported on Outlook
2003
without affecting, the current days or events already on it ?

Outlook will automatically populate Birthdays and Anniversaries if you
complete this information on your Contact's Details tab.

You don't - you use the contact's record to append the birthday to the
calendar.
