Cornell Method of Note-Taking Template using Ofc. 2007

I have been able to download the document but, I still have not been able to
create a Template of the Cornell Method of Note-Taking for me to save on my
computer then print out later.

I would appreciate any guidance on how to do it with this Word Document.

Two of the templates I find when searching office.microsoft.com for Cornell
are OneNote pages. Do you have OneNote? Or are you talking about the
PowerPoint template?
