Database design for Healthcare

Hello all,
We have been asked to do Access Database for health care program. The
program has two clinics (clinic one and clinic two)...Client-one gets
enrolled to clinic-one and Health Provider enters all demographics
information then...client-one gets initiall assessment which includes 10
different tools (for eg BMI, depression scale etc) after 3 month there is a
follow up then 6, 12, 18 month follow up. At this point either the client
gets discharged or gets enrolled to clinic 2. If dischared then it will mean
the client is cured or didn't want to contiue with the treatment. If client
has not showed progress...then he gets enrolled to clinic 2. In clinic-two he
goes through a different assessemnt every 3 month for the next 2 years. Then
gets discharged.
I guess my question is how should go about designing this? This is a program
also used for research and very important that we are able to generate
meaningfull data for the two clinic. The Health provider should be able to
get report displaying the outstanding tasks and the next follow up date for
the client as well as what more assessments are still waiting to be done. I
have tried to simplify it to make it easier to understand.
Any guidance is greatly appreciated.

If you're in the United States, are you aware of the stringent and very
detailed HIPAA patient data privacy laws? Or do you have some $50,000 to spare
to pay the fines for violating it? Some would say that Access is incapable of
providing sufficient security to meet these laws; others differ. But don't
dive into this hastily.

This is well beyond the scope of unpaid volunteer work on these newsgroups,
even discounting the HIPAA issue. I'd really suggest that you should find a
local Access (or other database) expert and prepare to pay for development, or
prepare to spend quite a bit of effort learning to use the database. Here's
