HOW: document template that shows a userform to select a singlerecord and complete merge

I have an excel spreadsheet and a word document template. When the
document template is used to create a new document, I want a userform
to pop up with the list of records. I want to be able to choose one
and have it merged into the current document. That's it.

Any idea how I do it?
