Have you tried:
http://www.irs.ustreas.gov/formspubs/index.html

IRS forms are available to download in *.pdf format. Usually the form can
be filled in using Adobe Reader 5.1 or later.

I did a quick look and found a 2004 version of your form.

Thanks Don
You can't print out the form using PC, you have to purchase pre printed
forms and fill them out.

Your statement is not correct.

1. Open the IRS *.pdf form in either Adobe Reader or Acrobat and then
print the form either before or after you have added your data to the form.
I have done this for at least three years for my Federal income tax forms. I
have also done the same for state taxes using *.pdf forms from the Michigan
Department of Treasury. I mention state, because it is likely that your
state has *.pdf versions of the state tax forms.

2. All IRS forms, instructions and publications can ordered at no cost
from the IRS via the IRS site. Many can be obtained free at USPS offices.

3. Most libraries in US have IRS forms and publications that can be
copied. However, there is usually a small per page cost.

By policy, the IRS accepts IRS forms printed from their *.pdf files.

Don,

While you are correct for the vast majority of forms, the 1099 series
is different. There is a 1099 pdf file available from the IRS, but if
you actually open it, you get a notice about a $50 fine per self
printed form you submit.

Here's the actual warning message from the first page of the 2005
1099-MISC form:

Attention: Do not download, print, and file Copy A with the IRS.

Copy A appears in red, similar to the official IRS form, but is for
informational purposes only. A penalty of $50 per information return
may be imposed for filing copies of forms that cannot be scanned.

You may order these forms online at Forms and Publications By U.S. Mail
or by calling 1-800-TAX-FORM (1-800-829-3676).

Here are the answers folks

1) *Yes.* You *can* download and print your own forms

2) *Yes.* The IRS *will* charge you a *$50 fine* for processin
*self-printed forms*. So you probably don't want to go with the abov
option, even though you *can*

3) *Yes.* You can order pre-printed 1099-MISC & 1096 forms for -*FREE*
from the IRS by visiting this link

http://www.irs.ustreas.gov/businesses/page/0,,id=23108,00.htm
(Allow *7* to *15* business days for the forms to arrive.

I hope this clears things up

**EDIT** LMAO - just realized how old this thread was originally. Bu
