merge list to envelope

The wizard has confused me. I had a little experience
with merge in other programs years ago, but now I am Word
only.

I have a list of names/addresses stored in one column in
a word document. Return at end of each line and extra
return between addresses.

I have an envelope with the return address and I can set
the <<address block>>, seemingly ok. I tried using the
wizard to no avail.

How do I set the source document? Is there a secret code?
I know there must be some code in the source, but what
does it look like? Help is not help.
