Outlook Sent Items Email

We have an end-user that on one particular day was sending out email both new
emails and reply emails and the emails show that the "from" address is
labeled as "unknown" and the emails were never delivered to recipients. What
could possibly cause this? We are currently using Google Apps mail client
with the google apps sync plug-in for outlook. Any comments and/or
suggestions are kindly welcome.
