problem creating form

Hi

I have set up a web site(fp 2003) and wish to have a page that includes a
form.I am using the guestbook template.I have followed all the directions to
set it up with the form results page set up.

When I test it out by puting in some text and pressing the submit button I
get a "404"error stating web page not found.

Can anyone help with some suggestions?

Was the form created in FP and published using FP in http mode to a server supporting the FP SE?
Note:
The guestbook is a special type of FP form and you can't add other fields to it
(if you want a custom form use the FP form wizard template, not the guestbook template)

If however you are trying to set up a Guestbook, this is worth a look, it
certainly avoids the spamming that goes with a standard Guestbook
Free Guestbook service http://www.smartgb.com

The page not found is /_derived/nortbots.htm

nortbots.htm only appears in five cases:

1) You are testing on a disc-based website, including a FrontPage or
Expression Web disc-based web that is being browsed through a
web-server.

2) You are testing on a website that does not have server extensions
installed or running.

3) The site (or form page) was FTPed from a disc-based web, or
published using a FTP address for the destination, or published to a
file system destination including a network share or UNC address (like
\\machinename\path\webname), or to an address like C:\sitename .
Publish must be to a URL of the form http://example.com

4) The site was correctly published, but the extensions were not
installed correctly or were mis-configured. Re-installing the
extensions may correct the problem. Some forms (example Search) may not
be supported by the server and may also show the error.

5) There is a bug associated with FrontPage98 which causes this
problem.

In your case I suspect 3) applies - the page was FTPed to the server.
Pages that use the FrontPage extensions MUST be published using HTTP,
never with FTP.

If FTP has been used the extensions may need to be repaired or
re-installed, but first try opening the remote site (
http://wazzasdotcom.freehostia.com ) in FrontPage and running
Tools->Recalculate Hyperlinks. If the extensions are not corrupted this
may correct the problem with the guestbook form.
