quiz in outlook with VBA

I need an information about proper approach to building quiz form i
outlook.

I need an e-mail message with 3 tabs

1. info about quiz (plain html, so that's no problem) at the end ther
should be a button that redirect to 2.
2. test form -one-choice questions using radiobuttons, and, again
button with redirection to 3.
3. containing form with input fields and checkboxes (from MS Wor
template)

after completion of 3. again a button with a confirmation o
completion, which send completed quiz back to me.

You have any suggeestion how to do this? VBA? maybe any templates?
