Why does an embedded pix become an attachment?

When I send an email by Outlook containing an embedded picture, the recipient
receives the email with both the image inside and as an attachment. How do I
keep from creating an attachment? Does it happen at my end, or is it a
security issue at the other end?

it's one attachment - displayed both in the message and as an attachment.
