access help desperately needed

I'm trying to create a form that has 3 fields that needs to be filled
manually each week. It's for a scorecard, and it was previously handled in
Excel

There are 20 Plants, and i'd like those plant names to be uneditable in the
form. Then there's fiscal week, which i'd like for the user to have to enter
it only once (i.e. 627), and all 20 records will have "627" filled
automatically in the FW column.

Then there'll be those 3 fields that will be updated manually

The problem is, i need that form to be able to be used each week, with the
old data stored as well...so it'll look something like this:

ENTER FISCAL WEEK: _______

PLANTS Manual1 Manual2 Manual3

A ________ _______ ________
B ________ _______ ________
C ________ _______ ________
D ________ _______ ________
E ________ _______ ________


And the form will be reusable, and the old data is still accessible.

Is there a way?

You are starting from the 'wrong' end you design the tables / relationships
within the db, before you start on the form
As I dont understand your requirement, with regard data, I would have
difficulty with a way forward.
Perhaps look here, which may give you some understanding, of what is
required.
Sketch out your data model on paper first.
