Action item tracking form with live email and calendaring

Context: managing status of action items for a team. Provide an action item
form in Word or Outlook, which allows you to put in the responsible parties'
names and deadlines for each action item. The form is integrated with
Outlook such that when this action item list is emailed, the to: address line
is automatically populated with each person's email address, each person's
calendar is automatically populated with the tasks, with follow up meetings,
periodic task reminders, etc.
