Adding in Stoplight indicators as a column to entry table

I am trying unsuccessfully to add in stoplight columns. I can get the
columns in, but am struggling with a logical expression that will give me the
correct results. All I want it to have it show green for 100% complete and
more than 5 days from due date. Yellow for less than 100% complete and less
than 5 days away. Red for anything with a finish date that has already
occurred.

Any help?
