Address missing from To: Message not sent

We are running Exchange 2000 and Outlook 2000 with all service packs applied.
Sometimes when a user sends an email to an email address (either a new email
or reply to an existing email) the email is sent to their Sent Mail folder
but the email address is missing from the "To:" If the email is CC or BCC'd
it is received by those email addresses. We have contacted the person who
the email is sent to (email address in the To:) and they did not receive it.
If you delete the email from the Sent Folder and check it in the Deleted
Items Folder the email says, "This message has not been sent". Has anyone
else run across this issue? If so, is their a resolution. I have found a MS
article advising not having 2 new email messages open at one time but not
sure if it entirely relates.
