Always prompts for username and password

Windows XP SP1
Outlook 2002 (10.6515.6626) SP3

When I click Send/Receive, or if I use a scheduled
Send/Receive to send or receive messages, Outlook prompts
me for a username and password for an account even though
the account has a saved password that worked in the past.

There are four POP3 email accounts configured within
Outlook. One is through Comcast and the other three are
through a personal ISP. The three ISP accounts all have
unique user IDs and passwords for each account, but they
all share the same mail configuration. Only one of those
three ISP accounts prompts me for a password, the other
two do not. I am able to send and receive mail through
this one account, only the password will not save.

What I have already tried:
1. System has been completed re-built: HDD formatted and
the OS and Office products individually re-installed with
the latest service packs
2. Removed the one problem account and re-added it
3. Set the one problem account as the default account
4. Changed the problem accounts incoming mail server to a
different server then the other two
5. In the Define Send/Receive Groups - Send/Receive
Group. In the Accounts box, I cleared the Receive Mail
Items check box on all of the duplicate accounts. (Per KB
281791)
6. Remove the user account information from the Protected
Storage System Provider subkey and re-entered my password
options. (Per KB 290684)

None of these things have resolved the issue. Please
help.
