Anybody have an "invoice" for member dues of nonprofit corp?

I am a member of a nonprofit, 501 (c) (3) corporation. We want to send a
letter to our current members. Contents would include thank you, highlights
of the year and include an "invoice" gently asking them to renew their
membership for the next year.

Anyone with ideas/samples would be appreciated.
