Attachments changing and deleting

Hi,

Whenever one of our users attaches multiple attachments e.g. .doc, .xls,
..pdf, outlook randomely changes the attachment structure. It deletes one
altogether and duplicates others in no particular pattern. The client is
Office XP fully service packed.

Any ideas?
