bring up a record to edit based on feilds selected- SQL help?

i have a form that asks the user how he wants to edit a record:

by name:
by reserve:
by inspection date:
by house number:

i need combo boxes to fill themselves witht he data from the tables for
name, reserve, house number, inpection date or w/e, so the user can browse
and pick say a name - then it would bring up the corresponding record to
edit.
(brings up the form filled out with that records info)
if they picked a reserve and isnpection date: multiple records would come
up.
(bring up a form that u can mosuewheel scroll through to see all the
records that showed up)
