Can you make a template for HCFA 1500 form. Thank you

How can I make the template or if you have any template ready. This is a
medicare form for billing. The form is already made providers have to fill
up the form. I need the exact measurments in WORD.

Thank you Herb. This is the exact form I was looking for. I know someone
bought the program for $50 and it was made in WORD using fields, so I decided
to make the form using the WORD but I had to make a lot of print out to find
the exact location. Still I would like to know how to make that in WORD for
the future in case if I make a new form.
