Creating an Index

This is my first attempt at index creation.
With my first attempt, I was able to get all instances of "John Smith" in
the document to show under "John Smith" in the index..
How do I get all these I need to show up as "Smith, John"?
How do I get all appearances of "Jane Smith" to be listed under "Jane" as a
sub-entry of "Smith"?
I would also like "Jane Harriet Smith" to be listed under "Smith, Jane"

To do this, you can't just select "John Smith" in text, press Alt+Shift+X,
and accept Word's version of the XE field. You must modify the field to say
"Smith, John." You can do this retroactively by displaying Hidden text (so
you can see the XE fields) and modifying them directly. If you have several
Smiths, you may want to create a two-level index, so that you have, for
example:

Smith
Jane Harriet (which would include Jane)
John
Mortimer

To do this, you use "Smith:John," "Smith:Jane Harriet," etc.

I was able - with a couple of minor adjustments - make it work for me.
