Custom Form link to Task?

I dont know if this is feasible, or at least simple. I have a custom contact
form with a custom field called "Client Report Due Date". I'm looking to
have a button, so when I click it, a TASK will be created with the Due Date
already filled in and hopefully some kind of reference to the Clients name in
the Subject line or notes section.

All the data entry is done in this custom contact form, and I'm looking to
save a step of creating a separate TASK.
