custom menu appears twice in menu bar

I've created a custom menu, which I insert into the menu bar just to the
right of the tools menu. This change is made and, and saved in, a
particular template.

When another user opens this template (in which the menu is saved), they get
two instances of the menu, right next to each other.

When I choose View | Toolbars | Customize, upon exiting the dialog, I also
get two instances of the menu, but only briefly (restarting word makes it go
away; switching to another Word document also sometimes makes it go away).

If I have the other user go into Customize, and apparently delete one of the
instances of the menu, they then become like me -- they normally only see
one instance of the menu, unless they go into the Customize dialog.

Going into the Organizer shows only one instance of the menu available for
copying. Looking at other templates does not show the menu existing
anywhere else (i.e., there isn't one copy in my template and another copy in
Normal).

Can anyone tell me why this is happening? Better yet, can anyone tell me
how to make it go away?
