custom or edit field in contact address book?! Help!!

Hi,

All I am trying to do is either change a couple of the
standard fields in an contact listing in the contacts
address book or add custom fields and have them show up
in the contact form when I'm entering a new contact...

Please help. This is very frustrating! Aargh.
