Can you add a spell check button to the menubar in Entourage?

No, there is no way to customize the toolbar. You can add shortcuts to
Entourage menu items. See the tutorial "Customize Entourage with Keyboard
Maestro"

You can select to automatically check spelling as you type. It's a
Preference under General Preferences --> Spelling. The little red squiggly
lines show up under misspelled words.
