Do I want to use fields?

Hey all,

I'm pretty sure it's possible to do this, only I'm not quite sure how.

I'd like to create a general Word document, a template so to speak,
where I would have a predetermined message and format. However, there
is information in the document that will be automatically filled in.

For example, I would like for the letter to have the name and address
of the receiver at the top. I won't know the name and address until I'm
ready to send it.

I'm going to have this information stored in a database, and other
fields which will be dollar values, have yet to be calculated and
inserted into this document.

I'm wondering if there's a way to open this document, pass it these
parameters, and have the fields be filled in with these values.

In addition, my application is web based, using mySQL and PHP. On
certain instances, my application would like to open/print a Word
document containing fields retrieved/calculated based on the client
that is open.

If anyone can explain how to do this, or refer me to some documentation
regarding this, I'd gladly appreciate it.
