e-mail sent from frontpage form show ip address - no name

I have created a html page in front page which is to e-
mail the results of the form input to a specified email
address. The results come through but you are supposed
to be able to see the senders e-mail address and username
(this is on an intranet) - all I get is the users ip
address. What am I doing wrong?
