E2004. Refuses to accept password as valid. OWA is okay...

This is something that has hit us on a couple of occasions. We've been
trying to pinpoint what's wrong here... and now we're close to the
assumption that it might just be a small pocket of evil in Entourage
2004...

Mac OS 10.3.7 / Entourage 2004 / Exchange 2003 (FE and BE).

The problem.
One of our users, who has previously been an Outlook:mac user under Mac
OS 9, is now using E2004. Since moving to Entourage, this user has
found that he is unable to send items, because his password gets
rejected.
This occurs, in spite of being able to connect to pick up mail, log-in
to the domain to pick up his Windows shares as well as being able to
connect to his mail through Outlook Web Access. It seems that the first
time Entourage polls the mail server (to send and receive items), the
password is rejected and from that point on continues to be rejected.

Here's a list of what we've tried.

Rebuild Entourage database.
Uninstall and reinstall Entourage.
Completely replaced the Mac G5 with an entirely new build of OS X and
apps (inc. Entourage).
Re-created the user's domain account (then re-imported email).
Tried the user's account on five other Macs.
Used a phony account and imported the user's PST.
Renamed the user's account.
Changed the user's password.

Basically, the only thing, out of all the above that worked was:

Installed the user's account on a PC.

Any ideas what might be the cause of this? It's very trying. As I've
mentioned, the user can still connect from his Mac to OWA, but that's
no substitute for a real mail client.
