Email Form Contents / Options

Please excuse the novice post, I have tried to read through the FAQ and

previous posts and this has been bugging me all day..

I need to create a form in HTML with check boxes to indicate when
sections have been completed. Once the user has checked the relevant
boxes I need the form to populate an email with the results and ideally

the distribution list(s) ready to be sent..


Any ideas would be greatly appreciated.. thanks Guys

Not possible w/ the default FP form
- you will need to learn server side scripting
