Entourage 2004 won't sync with Address Book

I'm having serious sync issues among iTunes, Entourage and Address Book.
Address Book will not populate with the contacts that are in Entourage after
resetting almost everything.

Can anyone help? Thank you.

iTunes plays no role in syncing Entourage to Address Book.
