Entourage "forgets" to include accounts on send/receive

I have 3 accounts in Entourage that are supposed to be included in the
send/receive action. Regularly, Entourage suddenly stops including one or
more of the accounts in the update, and I can't even access that account
individually. The only way to fix it is to Quit Entourage and restart it.

Does anyone know what might be causing this? I have a suspicion that it
might have to do with errors accessing the account. Maybe after an error,
Entourage simply flags the account as bad and stops including it?

If it's an exchange account this could be intentional behaviour.

Too many failed connections will cause exchange to lock out the client,
requiring a reset at the server. To avoid this, entourage (2004) will
'freeze' any account that has more than a few consecutive failures to
connect.
