Actually, the OP cross-posted to the mac.office.entourage newsgroup, where
I'm answering from.
If the CSV file was created in Windows or a web browser (which create
Windows Outlook-oriented CSVs), then the line-endings are wrong for the Mac
and it won't import anyway. Also most CSV files of that type have quotes
around every field, and Entourage will import all the quotes, creating a
mess (separate quotes around first and last name, etc.)
Where did this CSV file come from? If it was from Outlook, Windows Outlook
Express, or web browser like Yahoo, MSN, Earthlink, read on. If it was from
a Mac application, you could try making a copy, opening the copy in Excel
Mac, saving as Text (Tab-Delimited), and importing that Tab Text file. But I
think you might hot the same problems.

make a copy of the CSV file, and follow the instructions in the Installation
and Outlook sections of the ReadMe for converting from Outlook, Yahoo, etc.
