Entourage Quits when trying to switch identities

Entourage randomly quits when switching identities.

It randomly occurs with all 3 established identities. What will
happen, specifically, is that I will choose "switch identities" either
on the menu or by keyboard shortcut, and the dialog box to choose the
identities never appears. When I try again, it crashes.

I've slimmed down my databases and rebuilt them, both the simple and
advanced rebuilds, but this hasn't helped.

I've also trashed my Preferences for Entourage and run DiskWarrior.

To no avail, I'm still experiencing this behavior.

I get the same thing occassionally but eventually I'm able to switch
normally about 90% of the time.
