Entourage X (10.1.1) / Printing message headers (can't)

Hello,

I'm running OS 10.3.8, using an old HP Laserjet, model 6MP which has
worked fine....

Using the HP model-specific driver...

Problem: When I print an email message, the To/From/Date/Subject
header does not print, although the rest of the message does.

What also prints, and is weird, is a kind of header and footer like I
would use on my browser printouts, that is, date in one corner, page
name ("untitled") in another, the word "stream://1/" in another, and
"page 1 of 1" in the fourth corner. I have NOT set up these headers and
footers.

Is it a problem with Preview (which I find highly annoying)? Can I turn
it off?

This is a new and really annoying problem, but I can't date it to any
action on my part, probably in part because I rarely print emails.
Perhaps it has something to do with the recent updates to OSX?

I seem to recall a completely different look to my print preview
(fairly recently) that did include the header...

I think you'll find that HTML messages print this way. Text messages print
with the full header information. So far, there is no way to alter this
behavior.
