For some reason Entourage sends spam attachments to a "temp" file. How do I
set up a mailing list rule so it doesn't do that?

The Entourage Temp folder is just that. TEMPORARY. Anything that a MS app
puts in the Temp Folder should be deleted automatically when the app quits.

If you don't quit Entourage on a regular basis, you should run the script
"Delete Temporary Items folder" with a schedule.
