When custom labeling phone numbers in create contact box, once I
custom label a phone number, it changes all phone custom labels I
have already entered in the entire address book. How do I custom label
individual contacts without changing previous custom labels?

That's how Entourage works - the custom field labels apply across the
Address Book, they're not "per contact" as in Apple's Address Book. They're
real field labels. Why don't you just leave the label as "Custom 2" or
"Other" or whatever, and then add the unique individual "label" to the phone
number itself. E.g.:

Custom 2: (555) 333-1212 (country house)
