Exporting text from forms

Hi!
Anybody know how can I export the text of a form CONTAINING the las
entry in the drop down menu?

I'm a clinician, I made up a form to record session notes using man
drop down menus. I can save it in MS Word but exporting in ou
primitive software for progress notes is not working.
