Can someone point me in the direction of creating a simple form for my web
visitors - site created in FrontPage 2003 and hosted on Windows 2003 Server?

Try starting w/ the Forms wizard w/i your FP web - File New Page - more page templates
- requires a hosted site running the FP Server Extensions
(or IIS & FP SE under Windows 2003 Server)

the data to him?
