Getting info from records in a Table

Hello,

I am trying to make a form so people can purchase items. The list of items
and their price is in a table called Items.

My problem is how to load the prices for each item. They select which item
they want by using check boxes then choose delivery. The deliver is chosen by
a combo box and they have prices associated to them.

How do I get the price values into the form? I have tried some things with
the combo box but nothing seems to work, or the combo box displays the price
not description.
