Guestbook problem

I have a guestbook on my site www.oitcmusic.com/guestbook.htm that somehow
displays the form twice. It now even displays the buttons I created the
other day twice. I am not positive, but I believe this behavior started
after I upgraded to FP 2003 from FP 2000. This is so weird, if you browse to
the page, right click and choose view source, you can see the html code that
is duplicated, however if I open the guestbook page either locally or
remotely I do not see any of the duplicate code. The form still works
correctly when you submit a comment.

Any ideas what might cause this and how to fix it? I have also contacted my
webhost to see if it's possibly a FP SE problem. Haven't heard back from
them yet.

There are some bugs in the FP2002 SE related to shared borders & include pages
so make sure you (and your host) see & apply all appropriate patches from:
