Help with writing an "Appeal for letter grade"

I'm an undergraduate student. I need help and/or suggestion on writing an
"appeal for letter grade". I was informed to write a letter to the department
head if the instructor does not want to change the letter grade. Any help is
useful.

Well that's an interesting academic exercise for you then, isn't it? Or are
you reading some sort of mickey-mouse degree where English isn't required?
