How can I find a template or create one for daily fuel receipts?

I need to save/record daily fuel receipts, but the originals are czrbon and
do not copy or scan well.
How can I create a receipt form showing perhaps 5 days receipts that can
easily be filled in by me?

Publisher is not what you want for this task.
A much better application would be Excel which coud be set up quite
easily to add up the miles( Kilometres in the advanced world) and the
dollars.
