If you can't just click on the box, the form was badly designed (and
presumably isn't properly protected). Do whatever you have to do, up to and
including revising the form to work the way it should (I've often completely
redesigned bad forms I've been sent, though my improvements are never
acknowledged <g>).

Right-click on a blank section of your toolbar. In pop-up menu, select
"Forms." This will open the Forms toolbar. On the Forms toolbar you'll see a
check/tick box. Simply click that and a box will be inserted at your
cursor/insertion point.

There are a number of ways to create and use clickable checkboxes in
documents / templates. Some require protected or locked forms, others do
not. Take a look at the Checkbox template available at
http://www.addbalance.com/word/download.htm#CheckboxAddIn for an exploration
of these.

You might also want to look at the gender toolbar template on the same page.
